Assessment – We personally assess your loved one’s situation and care needs. A face to face meeting is always best, but we can also use phone, email and text. Our assessment will include type of care required, type of environment desired, timing of needed services, budget and financing, preferred location and specific family preferences.
Suggestions and Referrals – We will put you in touch with several care providers who meet your specific criteria. We will (with your permission) give your contact information to our suggested providers. We do not recommend, only refer. We will always suggest at least two providers and most often three to five. The final decision will be yours.
Visits – You will visit as many or as few of the referred care providers as you would like. We are happy to schedule visits for you and/or accompany you on your visits if helpful. We are available by phone, email or text to answer any and all questions. We will provide you with a checklist to use while visiting.
Evaluation and Follow Up – We will continue this process until we find the care provider that meets your needs. We will assist with any logistical details. We will follow up with the care provider and your family to make sure everything is as it should be in the weeks following any transition. Your feedback is most important to us.
Personal visits – We personally visit and evaluate every care provider we refer. We look for the specific criteria of each family we assist.
Licensing Criteria – We review all licensing inspection reports and look for any citations or complaints. We questions any citations.
Past History – We ask for feedback from families we have helped in the past. Their comments are our most important criteria.
